Consent to the processing of personal data

I hereby voluntarily, of my own free will, and in my own interests, give to NPA (hereinafter referred to as the Operator),

The list of personal data for which consent is given to the processing includes:

The purposes of processing personal data are:
to fulfill obligations that have arisen or may arise for the Operator during the execution of the agreement,

The above personal data will be processed using mixed (automated and non-automated) processing.

The Operator has the right to process my personal data in the following ways: collection, recording, systematization, accumulation, storage, clarification (updating, modification), transfer, use, depersonalization, blocking, deletion, and destruction of personal data.

I consent to the transfer of my personal data for the provision of services by the Operator:
